Families
of disabled children and young people might need to see a range of different health and care professionals at different stages of the child’s life.
The key worker service supports the named key worker and allows professionals in the different organisations and agencies to work together, in partnership with parents and carers, to ensure the child’s needs are met and that families have the services and support they need.

Parents can make a referral by contacting Sky Smith on 07586 731817 (Monday, Wednesday and Thursday) or 01603 508958 or emailing sky.smith@nchc.nhs.uk
Referrals should be made on the NCHC referral form and sent to our Children’s Single Point of Referral team, this is a single point of entry to all of NCHC’s children’s services.
