Opting Out of School Screening Services

The National Child Measurement Programme (NCMP) measures the height and weight of children in Reception and Year 6 in schools across England. Vision and hearing screening during reception is an opportunity to pick up any problems that have not already been identified.

  • Hearing screening - Reception Year ONLY.

It is important that you opt your child out of hearing screening if they have a programmable shunt fitted – the equipment we use can affect the programable shunt.

You will receive a letter via your child's school letting you know when the screening is happening. If you would like to opt your child out, or if you would like to supply additional information, please ensure the form below is submitted before the deadline date on the letter. If you are opting out after the deadline date please call Just One Number 0300 300 0123 to discuss.

If you are happy for your child to be seen, you do not need to do anything.

Providing more information before the visit

If you need to provide us with more information before your child's screening, please complete part 1 and part 3 of the form and submit. Do not complete part 2.
