16-19 Health Passport App

As young people become more independent it is important that they become more confident about caring for their own health and wellbeing.

The Health Passport App has been designed to support this. The app provides general health information and advice on a variety of topics and helps Norfolk young people find up to date information, advice and support on the health topics that affect them.

What Else Does The App Do?

Young people can register through their GP practice to access information from their their own health record. This is located via the “My health record” tile on the main screen. 

To request access to your record, please speak to your GP practice.
