16-19 Health Passport App

As young people become more independent it is important that they become more confident about caring for their own health and wellbeing.

The Health Passport App has been designed to help with this. The app helps Norfolk young people find up to date information, advice and support on the health topics that affect them.

The app provides general health information and advice on a variety of topics! 

Young people can register through their GP practice to access information from their their own health record. This is located via the tile  “My health record”.
