Personalised care means that people have control and choice over the way their care is planned based on what matters to them, and their individual needs.
A Personal Care and Support Plan (PCSP) is the result of discussions with your midwife or obstetrician (a doctor specialised in care during pregnancy and childbirth) about you, to build up a picture of your life and how this could affect your plans for pregnancy and childbirth.

Some hospitals provide personalised care and support planning booklets and others use an app where you can note down any thoughts, feelings and questions that you would like to ask.
